*I/we understand this form and the backup material are due in the superintendent’s office by 12:00 noon on the Monday (agenda items) and Tuesday (supporting documents) prior to the scheduled board meeting

(Note: It will be the requesting Administrator’s responsibility to make sure all other administrators are involved if this request will affect the total district.)
